- Assessment and planning of NDIS participants’ needs, goals, and aspirations, with a focus on individualisation and person-centeredness.
- Coordination and management of supports and services to ensure that they are meeting the participant’s needs, and that their NDIS plan is being implemented effectively.
- Ongoing monitoring, review and adjustment of the participant’s NDIS plan and supports, to ensure that they remain appropriate and effective over time.
